Williams County birth/death Certificates

The Williams County Health District maintains birth and death records occurring in Williams County beginning in 1909 to present.  If the death occurred outside of Williams County, the record may be obtained from the Ohio Department of Health Vital Statistics Office for deaths occurring from 1964 to present or the Ohio Historical Connections for deaths occurring from 1908 to 1963.  You may also obtain death records from the county in which the event occurred.  We are able to issue certified birth certificates for anyone born in the state of Ohio.

How to obtain birth/death certificates

Certified copies of birth and death records may be obtained in the following manner for $25.00 each:

We accept cash, check or money order for same day service.

Please download the birth/death application or send the following information:

1.  Birth information:  name at birth, date of birth, mother’s maiden name, father’s name and where birth occurred.
2.  Check or money order for $25.00 for each certificate requested made out to the Williams County Health District.
3.  A self addressed, stamped envelope.
4.  Daytime phone number where you can be reached for questions.

Orders are processed the day they are received.
