Program Goal
The primary goal of the Caregiver Resource Center is to create a central resource point in Williams County for caregivers to get education, support, and resources necessary to effectively assist them in their challenging role.
Case Management Services
A unique case management program provides a registered nurse to do in-home assessments of caregiver needs. The RN offers resources, advice on caregiving, and provides:
Caregiver Resource Library
A community library for caregiver materials including pamphlets, books, videos, current magazines, and disease specific information located at the Williams County Senior Center, 621 East Main St., Montpelier, Ohio 43543, 419-485-3218.
For in-home assessments and consultation, call the Caregiver Coordinator at the Williams County Health Department 419-485-3141 or 419-636-4004, Monday through Fridays 8:30-4:30 p.m.
Some Reasons For Calling:
© Caregiver exhaustion, illness, increase in stress, depression, anger, or
safety concerns.
© Patient with behavior problems such as wandering, outbursts,
agitation, depression, or combativeness.
© Strained family relationships or finances.
© Family meeting needed to address options for care.
