What is Help Me Grow?
Help Me Grow provides services to pregnant women and children in the first three years of life and includes home visits by a registered nurse and screenings for health, hearing, vision, and child development.
Who is Eligible for Help Me Grow Services?
All first time moms and all teen moms. They can receive a visit from a nurse to help in the transition time of taking care of a new baby. A nurse will contact these moms for a visit 4-6 weeks after delivery. During a visit, the nurse conducts a checkup of the mother and baby and shares information about health, development, and safety (see below). Referrals to other community services are also offered. Also, a prenatal visit is available after the 8 month check-up. Please call for an appointment for this visit at 419-485-3141 or 419-636-4004. There is no cost to this program and many families find this visit very useful.
Any child, from birth to three years old may be determined eligible based on the following:
* If an evaluation of your child's development is shown to be delayed,
* If the child has a medically diagnosed physical or mental condition that has a high chance of resulting in a development delay.
* If your child has four or more risk factors present in the child and/or family, which could interfere with caregiving, health or development of the child
