Food service operation/retail food establishment-license required (ORC

  3717.21 or ORC 3717.41) except as provided in ORC 3717.22 or ORC 3717.42,

  no person or government entity shall operate a food service operation/retail food

  establishment with out a license.  A separate license is required for each food

  service operation/retail food establishment a person or government entity

  operates. No person or government entity shall fail to comply with any other

   requirement of this chapter applicable to food service operation/retail food

   establishment.
