

Changing ownership, altering, or constructing a food service operation/retail food establishment - Completed facility layout review packet must be submitted to the health department. The health department will review the information and issue a letter of plan approval or request for more information within 30 days of receipt. The final approval letter will include risk level classification, fee requirements, and an application. The application along with the appropriate fee must be submitted to the health department 10 days prior to opening. A pre-opening inspection must also be scheduled at that time. If the food service operation/retail food establishment conforms to the previously approved facility layout review at the time of the pre-opening inspection, the license will be issued. The license must be displayed at the licensed location at all times (OAC 901:3-4-02(B)(C)(D)(E)(G) or OAC 3701-21-02(B)(D)(E)(F)(H)
Temporary food service operation/retail food establishment – Completed application and appropriate fee must be submitted to the health department 10 days prior to the event. If event is held in another county, please contact that county health department for application and fee information. A temporary food service operation license expires at the end of the period for which it was issued and is not renewable. A maximum of 10 temporary food service operation licenses may be issued to a person or government entity for various events within a licensing period and only one temporary food service operation license may be issued to the same person or government entity per event (ORC 3717.43(C)(D)(E) or ORC 3717.23(C)(D)(E)). A temporary food service operation/retail food establishment inspection will be conducted at the event during operation and the license will be issued at that time. The license shall be displayed at the operation at all times (ORC 3717.43(G) or ORC 3717.23(G)).
