As a new school year approaches, now is a great time to make sure your child is up-to-date on their immunizations. Staying current on recommended and required immunizations helps protect your child, their classmates, and the community from vaccine-preventable diseases.
WCHD Back to School Vaccine Clinics
Williams County Health Department is offering special immunization clinics at local schools this fall to help students get up to date on required vaccinations before the school year gets underway.
Appointments are required. Parents and guardians can schedule online using the school-specific booking links below. After scheduling, you will receive a text message with consent forms and health screening questions that must be completed before the appointment.
If online scheduling is not convenient, you may also call to schedule an appointment. Please note that the required consent forms will still need to be completed electronically before the appointment.

Back to School Vaccines
Ohio law requires students to be protected against 10 vaccine-preventable diseases: Diptheria, hepatitis B, measles, meningococcal diseases, mumps, pertussis (whooping cough), polio, rubella, tetanus, and varicella (chickenpox). Specific immunization requirements apply based on a student’s grade level.
Children entering Kindergarten, 7th grade, and 12th grade should have their immunization records reviewed to make sure they have received all required vaccines for their grade level. Other students may also need to catch up on missed or overdue immunizations
Before the school year beings, check with your child’s pediatrician or healthcare provider to review their immunization record. If your child needs vaccines, schedule an appointment early to help avoid the back-to-school rush.

The Williams County Health Department offers vaccinations to families with private health insurance, Medicaid, and those without health insurance. Your health insurance company, including Medicaid, will be billed for the vaccination services. Children without health insurance or whose insurance does not cover vaccinations can receive a discount on the vaccination services, if eligible. We will not turn anyone away due to an inability to pay.
The Day of Your Appointment
For your appointment, bring with you:
- Your vaccination record if you’ve received vaccines outside of Ohio or if you have a paper copy of a record (your doctor can fax records to 419-216-5108)
- Your insurance card, if you have one.
- Third Party Consent Form, if needed
