Birth and Death Certificates

The Williams County Health Department maintains birth and death records occurring in Williams County that began in 1909 to present. This Vital Statistics office is able to issue certified birth certificates for anyone born in the state of Ohio. We have access to all birth and death records filed in Ohio after December 20, 1908.

How do I request a birth certificate?

Williams County Health Department can issue birth certificates to anyone born in the state of Ohio.    You can make a request by phone, mail, online, or in person at our office. Home births are also registered by the Health Department.

The information we need to complete your request includes:

  1. Name at birth on the certificate
  2. Date of birth on the certificate
  3. Maiden name of the mother
  4. Father’s name, if applicable
  5. The city and county where the birth occurred

Please complete this the Application for Birth Certificate. Print and attach the appropriate fee ($25 for each certificate).  Orders are processed the day they are received.

Or send the following information:

  1. Birth information: name at birth, date of birth, mother’s maiden name, father’s name (if applicable) and where the birth occurred
    2. Check or money order for $25.00 for each certificate requested made out to “Williams County Health Department”
    3. A self-addressed, stamped envelope
    4. Daytime phone number where you can be reached for questions

The birth request will go to the county where the person was born. Be advised we cannot provide status of an order of someone not born in Williams County (example: if you were born in Williams County, we will get your request, if you were born in Columbus, Franklin County will get your request).

How do I request a death certificate?

In addition to birth certificates, we also register all deaths that occurred in Williams County. Death certificate requests in which the death occurred in another county must be obtained through the county of death.  If the death occurred outside of Williams County, the record may be obtained from the Ohio Department of Health Vital Statistics Office for deaths occurring from 1964 to present or the Ohio Historical Connections for deaths occurring from 1908 to 1963.

Genealogy Searches

The Ohio Department of Public Health’s Bureau of Vital Statistics maintains birth records from December 20, 1908 and death records from 1971 to present. Fetal death records, heirloom birth certificates, and stillbirth certificates can also be ordered. Marriage and divorce records are NOT maintained by the Bureau of Vital Statistics. Certified copies of marriage licenses and divorce decrees can only be obtained from the county where the event was recorded.

  • Marriage certificate copies can be obtained from the specific county probate court. In Ohio, each county probate court maintains their own online contact pages and information.
